Staffing a clinic when everyone is part-time

Rotas built in a spreadsheet break the first time someone swaps a shift. A better model treats availability, rooms, and skills as one schedule.

The modern practice is a patchwork: two clinicians on Mondays, one on Thursdays, a nurse who covers two sites, a locum every other Friday. The rota that holds it together is almost always a spreadsheet, and the spreadsheet is almost always wrong by Wednesday.

The rota is not the schedule

Most practices keep two documents: who is working, and what is booked. They are edited by different people, at different times, in different tools. The gap between them is where the double-booked room and the empty clinic both come from.

The fix is not a better spreadsheet. It is to stop keeping two documents. Availability, rooms, and skills belong on the same calendar as the appointments they make possible.

Three constraints, one view

  1. People. When someone is in, and which appointment types they can take.
  2. Rooms. Which spaces exist, and what each one is equipped for.
  3. Skills. A minor procedure needs a clinician who does minor procedures and a room with the kit — at the same time.

When all three live on one schedule, a swapped shift is not an email thread. It is a drag, and the calendar tells you immediately which bookings no longer have a person or a room.

Design for the swap

Part-time teams swap constantly, and every swap is a small risk. Handle it with a rule rather than a conversation: a swap is offered to everyone with the same skill set, the first to accept takes it, and any appointment that becomes uncovered goes to the waitlist for rebooking. What to measure

  • Uncovered hours — booked time with no matching person or room.
  • Idle rooms — equipped rooms with nobody rostered.
  • Swap latency — hours from a swap request to a confirmed cover.
